Da je prekomjerna težina važan rizičan faktor povezan s težim slučajevima COVID-a uvjerili su se hrvatski liječnici koji rade u jedinicama intenzivnog liječenja, a službeno to potvrđuju i najnovija istraživanja. Znanstvenici s Oxforda otkrili su da rizik od lošijih ishoda COVID-19 počinje rasti kod ljudi s indeksom tjelesne mase (engl. body mass index, BMI) iznad 23 kilograma/m2. Učinak prekomjerne tjelesne težine na rizik od komplikacija najveći je kod mladih ljudi u dobi od 20 do 39 godina, a smanjuje se nakon 60-te.

Rizici hospitalizacije su 5 posto veći za svaki porast jedinice indeksa tjelesne mase, a rizik od prijema na intenzivnu linearno raste 10 posto za svako povećanje ITM-a.

Kod ljudi starijih od 80 godina taj rizik značajno opada, odnosno debljina ima vrlo malo utjecanja na ishod bolesti.

Studija je u četvrtak objavljena u znanstvenom časopisu The Lancet Diabetes & Endocrinology, temelji se na analizi zdravstvenih kartona više od 6,9 milijuna ljudi u Velikoj Britaniji, a obuhvatila je podatke od preko 20 000 pacijenata s COVID-19 koji su bili hospitalizirani ili umrli tijekom prvog vala pandemije u zemlji.

Ako pretilost mjerimo indeksom tjelesne mase optimalna tjelesna težina iznosi 18.5 do 24.9 kg/metru četvornom, dok je prekomjerna tjelesna težina 25 do 29.9 kg/m2, a pretilost iznad 30 kg/m2.

Primjerice osoba visoka 180 cm i teška 70kg ima BMI 21.6, a osoba iste visine teška 90 kilograma BMI 27.8. Pretilost bi bila kada bi ova osoba imala 100 kg i više.

"Naše istraživanje pokazuje da je čak i vrlo skromna prekomjerna tjelesna težina povezana s većim rizicima od teških komplikacija COVID-19 te da rizici naglo rastu kako BMI raste", navela je prof. Carmen Piernas, vodeća autorica studije sa Sveučilišta Oxford. 

- Studija također pokazuje da su rizici povezani s prekomjernom tjelesnom težinom najveći kod osoba mlađih od 40 godina, dok težina ima malo ili nimalo utjecaja na šanse da se razvije teški COVID nakon 80-te godine - dodala je. 

I brojna prethodna istraživanja došla su do zaključka da je pretilost povezana s težim ishodima nakon infekcije virusom SARS-CoV-2, ali ovo je prvo koje je ispitalo posljedice prekomjerne težine u cijelom rasponu indeksa tjelesne mase .

Ova otkrića znanstvenika s Oxforda sugeriraju da politike cijepljenja trebaju dati prioritet osobama s pretilošću, pogotovo što se cjepivo uvodi u mlađe dobne skupine.

U SAD-u 40 posto pretilo u Hrvatskoj 20

Sam indeks tjelesne mase nije savršena mjera, jer će mišićavije osobe imati viši indeks tjelesne mase, premda će udio masnog tkiva biti relativno nizak, tako da je pri procjeni rizika od razvoja bolesti povezanih s pretilošću važno uzeti u obzir i neke druge mjere koje ukazuju na raspodjelu masnog tkiva u tijelu poput odnosa opsega struka i bokova. Takav izračun ukazuje na stupanj rizika od razvoja nekih bolesti poput visokog krvnog tlaka, šećerne bolesti, poremećaja razine masnoća u krvi, krvožilnih bolesti uključujući srčani i moždani udar, žučnih kamenaca, osteoartritisa, nekih vrsta raka i drugih. Što je indeks viši, to je i veći rizik pobolijevanja od navedenih bolesti.

Prema zdravstvenim kartonima više od 40 posto odraslih osoba u SAD-u je pretilo, dok je u Hrvatskoj oko 20 posto.

Da je prekomjerna težina važan rizičan faktor povezan s težim slučajevima COVID-a uvjerili su se hrvatski liječnici koji rade u jedinicama intenzivnog liječenja, a službeno to potvrđuju i najnovija istraživanja. Znanstvenici s Oxforda otkrili su da rizik od lošijih ishoda COVID-19 počinje rasti kod ljudi s indeksom tjelesne mase (engl. body mass index, BMI) iznad 23 kilograma/m2. Učinak prekomjerne tjelesne težine na rizik od komplikacija najveći je kod mladih ljudi u dobi od 20 do 39 godina, a smanjuje se nakon 60-te.

Rizici hospitalizacije su 5 posto veći za svaki porast jedinice indeksa tjelesne mase, a rizik od prijema na intenzivnu linearno raste 10 posto za svako povećanje ITM-a.

Kod ljudi starijih od 80 godina taj rizik značajno opada, odnosno debljina ima vrlo malo utjecanja na ishod bolesti.

Studija je u četvrtak objavljena u znanstvenom časopisu The Lancet Diabetes & Endocrinology, temelji se na analizi zdravstvenih kartona više od 6,9 milijuna ljudi u Velikoj Britaniji, a obuhvatila je podatke od preko 20 000 pacijenata s COVID-19 koji su bili hospitalizirani ili umrli tijekom prvog vala pandemije u zemlji.

Ako pretilost mjerimo indeksom tjelesne mase optimalna tjelesna težina iznosi 18.5 do 24.9 kg/metru četvornom, dok je prekomjerna tjelesna težina 25 do 29.9 kg/m2, a pretilost iznad 30 kg/m2.

Primjerice osoba visoka 180 cm i teška 70kg ima BMI 21.6, a osoba iste visine teška 90 kilograma BMI 27.8. Pretilost bi bila kada bi ova osoba imala 100 kg i više.

"Naše istraživanje pokazuje da je čak i vrlo skromna prekomjerna tjelesna težina povezana s većim rizicima od teških komplikacija COVID-19 te da rizici naglo rastu kako BMI raste", navela je prof. Carmen Piernas, vodeća autorica studije sa Sveučilišta Oxford. 

- Studija također pokazuje da su rizici povezani s prekomjernom tjelesnom težinom najveći kod osoba mlađih od 40 godina, dok težina ima malo ili nimalo utjecaja na šanse da se razvije teški COVID nakon 80-te godine - dodala je. 

I brojna prethodna istraživanja došla su do zaključka da je pretilost povezana s težim ishodima nakon infekcije virusom SARS-CoV-2, ali ovo je prvo koje je ispitalo posljedice prekomjerne težine u cijelom rasponu indeksa tjelesne mase .

Ova otkrića znanstvenika s Oxforda sugeriraju da politike cijepljenja trebaju dati prioritet osobama s pretilošću, pogotovo što se cjepivo uvodi u mlađe dobne skupine.

U SAD-u 40 posto pretilo u Hrvatskoj 20

Sam indeks tjelesne mase nije savršena mjera, jer će mišićavije osobe imati viši indeks tjelesne mase, premda će udio masnog tkiva biti relativno nizak, tako da je pri procjeni rizika od razvoja bolesti povezanih s pretilošću važno uzeti u obzir i neke druge mjere koje ukazuju na raspodjelu masnog tkiva u tijelu poput odnosa opsega struka i bokova. Takav izračun ukazuje na stupanj rizika od razvoja nekih bolesti poput visokog krvnog tlaka, šećerne bolesti, poremećaja razine masnoća u krvi, krvožilnih bolesti uključujući srčani i moždani udar, žučnih kamenaca, osteoartritisa, nekih vrsta raka i drugih. Što je indeks viši, to je i veći rizik pobolijevanja od navedenih bolesti.

Prema zdravstvenim kartonima više od 40 posto odraslih osoba u SAD-u je pretilo, dok je u Hrvatskoj oko 20 posto.
