Profesor Robert Likić s Medicinskog fakulteta u Zagrebu gostovao je u N1 studiju uživo i komentirao situaciju s pandemijom koronavirusa u Hrvatskoj.
Likić je rekao da treba revidirati procjene dovoljne razine procijepljenosti stanovništva da bi se postigao kolektivni imunitet.
“Prve procjene oko postotka procijepljenosti da bi se postigao kolektivni imunitet zasnivali su se o podacima zaraznosti prvog soja koronavirusa, nastalog u Wuhanu u Kini. U međuvremenu pojavilo se nekoliko varijanti toga virusa, od kojih je u Europi inicijalno dominirala britanska varijanta, koja je bila zaraznija, a sada dominira ili počinje dominirati delta varijanta.
Pojavila se prvo u Indiji i za nju je karakteristično da je ona još oko 60 posto zaraznija u odnosu na britansku varijantu, koju smo mi imali dominantnu inicijalno u Velikoj Britaniji pa u ostalim zemljama Europske unije i kod nas. Zaključno je indijska varijanta u odnosu na britansku oko 200 posto zaraznija. Ova razina od 67-70 posto procijepljenosti bila je potrebna za originalnu varijantu, zaraznije varijante trebaju veću procijepljenost”, rekao je Likić.
Pozvao se i na glavnog američkog epidemiologa Anthonyja Faucija, koji također više ne govori o 70 posto procijepljenosti, nego sugerira da treba težiti što višem postotku cijepljenih građana.
Na pitanje koliki bi taj postotak sada trebao biti, Likić za N1 odgovara:
“Ja bih rekao barem preko 85 posto, ako ne i 90 posto. Zato što je virus zarazniji u odnosu na prvi soj i jednako tako veći postotak zaraženih ljudi treba bolničko liječenje. Kod originalnoga virusa taj postotak se kretao oko 10 posto, kod indijske varijante je oko 20 posto.
Ako budemo imali lošiju procijepljenost, ona je sad oko 50 posto, to znači da će se delta varijanta, koja će uskoro postati dominantna i u Hrvatskoj, uglavnom širiti među necijepljenom populacijom. Ako uzmemo u obzir da će 20 posto necijepljene populacije po procjenama trebati bolničko liječenje, mi bismo mogli imati vrlo značajan četvrti val sa značajnim socioekonomskim posljedicama koje idu uz to. Trebali bismo ići s edukacijama da se potakne ljude da se što više cijepe”, rekao je Likić.
On se ne slaže s tvrdnjama da je klinička slika zaraženih delta varijantom koronavirusa blaža.
“Ona je blaža ako je zaražena osoba cijepljena barem jednom, idealno s dvije doze cjepiva. Uistinu možemo računati da će za 20 posto neprocijepljene populacije biti potrebno bolničko liječenje i s tog aspekta trebalo bi raditi na tome da se određeni postotak populacije cijepi”, rekao je Likić, ponovivši da inicijalni cilj od 67 do 70 posto cijepljene populacije za postizanje kolektivnog imuniteta treba biti povećan.
“Provodili smo istraživanja, to se još uvijek provodi, u tijeku je u SAD-u veliko istraživanje učinkovitosti primjene antidepresiva Fluvoxamina u sprečavanju pogoršanja stanja u izvanbolničkih pacijenata i preliminarni rezultati što naši što iz SAD-a, gdje su napravljena dva manja istraživanja, pokazuju da ljudi koji su liječeni Fluvoxaminom nisu trebali bolničko liječenje. Dakle, što se prije počne terapija, to je ona učinkovitija. Nisu trebali bolničko liječenje i nisu imali lošije ishode. Mehanizam je sprečavanje sinteze i oslobađanje citokina i nastajanja citokinske oluje za koju se smatra da dovodi do pogoršanja stanja kod pacijenata”, objasnio je Likić.
Na pitanje kad bi se taj lijek mogao početi šire primjenjivati u liječenju covida, Likić je rekao da sve ostaje na dogovoru liječnika i pacijenta.
“U načelu, uz taj lijek, a to bi bila prenamjena toga lijeka, farmaceutska industrija ubrzano radi i očekujemo dosta dobrih vijesti u osmom i devetom mjesecu. Dotad ostaje na dogovoru liječnika i pacijenta da koriste taj lijek. Ako pretpostavimo da je kod delta varijante zaraznost 20 posto, tu bi to vrijedilo razmatrati”, rekao je Likić.

Profesor Robert Likić s Medicinskog fakulteta u Zagrebu gostovao je u N1 studiju uživo i komentirao situaciju s pandemijom koronavirusa u Hrvatskoj.
Likić je rekao da treba revidirati procjene dovoljne razine procijepljenosti stanovništva da bi se postigao kolektivni imunitet.
“Prve procjene oko postotka procijepljenosti da bi se postigao kolektivni imunitet zasnivali su se o podacima zaraznosti prvog soja koronavirusa, nastalog u Wuhanu u Kini. U međuvremenu pojavilo se nekoliko varijanti toga virusa, od kojih je u Europi inicijalno dominirala britanska varijanta, koja je bila zaraznija, a sada dominira ili počinje dominirati delta varijanta.
Pojavila se prvo u Indiji i za nju je karakteristično da je ona još oko 60 posto zaraznija u odnosu na britansku varijantu, koju smo mi imali dominantnu inicijalno u Velikoj Britaniji pa u ostalim zemljama Europske unije i kod nas. Zaključno je indijska varijanta u odnosu na britansku oko 200 posto zaraznija. Ova razina od 67-70 posto procijepljenosti bila je potrebna za originalnu varijantu, zaraznije varijante trebaju veću procijepljenost”, rekao je Likić.
Pozvao se i na glavnog američkog epidemiologa Anthonyja Faucija, koji također više ne govori o 70 posto procijepljenosti, nego sugerira da treba težiti što višem postotku cijepljenih građana.
Na pitanje koliki bi taj postotak sada trebao biti, Likić za N1 odgovara:
“Ja bih rekao barem preko 85 posto, ako ne i 90 posto. Zato što je virus zarazniji u odnosu na prvi soj i jednako tako veći postotak zaraženih ljudi treba bolničko liječenje. Kod originalnoga virusa taj postotak se kretao oko 10 posto, kod indijske varijante je oko 20 posto.
Ako budemo imali lošiju procijepljenost, ona je sad oko 50 posto, to znači da će se delta varijanta, koja će uskoro postati dominantna i u Hrvatskoj, uglavnom širiti među necijepljenom populacijom. Ako uzmemo u obzir da će 20 posto necijepljene populacije po procjenama trebati bolničko liječenje, mi bismo mogli imati vrlo značajan četvrti val sa značajnim socioekonomskim posljedicama koje idu uz to. Trebali bismo ići s edukacijama da se potakne ljude da se što više cijepe”, rekao je Likić.
On se ne slaže s tvrdnjama da je klinička slika zaraženih delta varijantom koronavirusa blaža.
“Ona je blaža ako je zaražena osoba cijepljena barem jednom, idealno s dvije doze cjepiva. Uistinu možemo računati da će za 20 posto neprocijepljene populacije biti potrebno bolničko liječenje i s tog aspekta trebalo bi raditi na tome da se određeni postotak populacije cijepi”, rekao je Likić, ponovivši da inicijalni cilj od 67 do 70 posto cijepljene populacije za postizanje kolektivnog imuniteta treba biti povećan.
“Provodili smo istraživanja, to se još uvijek provodi, u tijeku je u SAD-u veliko istraživanje učinkovitosti primjene antidepresiva Fluvoxamina u sprečavanju pogoršanja stanja u izvanbolničkih pacijenata i preliminarni rezultati što naši što iz SAD-a, gdje su napravljena dva manja istraživanja, pokazuju da ljudi koji su liječeni Fluvoxaminom nisu trebali bolničko liječenje. Dakle, što se prije počne terapija, to je ona učinkovitija. Nisu trebali bolničko liječenje i nisu imali lošije ishode. Mehanizam je sprečavanje sinteze i oslobađanje citokina i nastajanja citokinske oluje za koju se smatra da dovodi do pogoršanja stanja kod pacijenata”, objasnio je Likić.
Na pitanje kad bi se taj lijek mogao početi šire primjenjivati u liječenju covida, Likić je rekao da sve ostaje na dogovoru liječnika i pacijenta.
“U načelu, uz taj lijek, a to bi bila prenamjena toga lijeka, farmaceutska industrija ubrzano radi i očekujemo dosta dobrih vijesti u osmom i devetom mjesecu. Dotad ostaje na dogovoru liječnika i pacijenta da koriste taj lijek. Ako pretpostavimo da je kod delta varijante zaraznost 20 posto, tu bi to vrijedilo razmatrati”, rekao je Likić.
