Iako s ovako visokim temperaturama zraka još i ne pomišljamo na jesen, Saša Srića, pulmolog iz zagrebačke Klinike za plućne bolesti Jordanovac, upozorava da će vrlo brzo krenuti jesenske respiratorne infekcije i poziva građane na cijepljenje protiv gripe.

- Onaj tko u isto vrijeme oboli od gripe i COVID-a može imati teške posljedice. Čak i za one ljude koji su u punom zdravlju, kombinacija te dvije bolesti u isto vrijeme ne bi imala lagan tijek. Gripa također može biti teška akutna bolest s komplikacijama i smrti, iako za razliku od COVID-a ne ostavlja trajne posljedice na plućima, poput plućne fibroze - veli Srića.

Naš sugovornik posebno je zabrinut činjenicom što je u odnosu na 24. kolovoza povećan broj pacijenata koji se liječe u bolnicama od COVID-a za gotovo 100 posto, te što je trenutno 26 najtežih pacijenata na respiratoru, dok ih je koncem kolovoza bilo upola manje - 12.

Ima li osoblja?

- Ima i mladih na respiratoru, nekih čak koji su imali vrlo blage prethodne bolesti, ali se sada bore za život. Na raspolaganju imamo dovoljno respiratora, mislim da ih je ukupno više od 700 u cijeloj zemlji, no pitanje je hoćemo li imati dovoljno zdravstvenog osoblja da skrbi o svim oboljelima ako se nastavi trend rasta - napominje liječnik.

Prema statistikama, većina pacijenata zaraženih COVID-om ima blage simptome bolesti, dok 10 do 15 posto teških slučajeva zahtijeva hospitalizaciju i oksigenoterapiju. Od toga oko pet posto najtežih pacijenata završava na mehaničkom respiratoru, dok srednje teški slučajevi dobivaju kisik u visokim protocima kroz nosne cjevčice. Iako stalno slušamo o respiratorima, na sreću, relativno malo oboljelih je na svojoj koži osjetilo što se događa kad ste priključeni na ovaj stroj.

- Na respiratore se stavljaju najteži pacijenti kojima pluća zbog bilateralne intersticijske pneumonije ne mogu samostalno raditi, odnosno ne mogu više dostaviti dovoljno kisika u krv jer su pluća ispunjena tekućinom i upalom. To onda radi respirator. Bolesniku se pod pozitivnim tlakom preko respiratora dostavlja kisik, a odstranjuje višak ugljičnog dioksida. Kisik koji mi dišemo je u koncentraciji od 21 posto, a na respiratoru se može namjestiti i na 100 posto - objašnjava Srića.

Respirator nije uređaj poput CT-a ili MR-a, gdje ste svjesni svega što se događa. Osobe priključene na respirator su uspavane sve dok ih liječnici ne probude i procijene da mogu samostalno disati. Cijelo vrijeme primaju lijekove protiv bolova, na aparatu za disanje ostaju oko 14 dana, a s njihovim oporavkom treba biti vrlo strpljiv.

- Da bi se oboljeli mogao priključiti na respirator prije ga je nužno intubirati, odnosno uvesti kroz nos ili usta cjevčicu koja je priključena na uređaj. Osoba se prvo uspava, nakon čega mu se cijev provede do plućnih krila. Mnogi znaju reći, kad završiš na respiratoru to je znak da si gotov i da možeš poručiti: "Zbogom! No, nije to baš tako, respirator je spasio živote velikom broju oboljelih – dodaje Srića.

Prilično zbunjeni

Stopa preživljenja pacijenata na respiratoru varira ovisno o dobi pacijenta te njihovim komorbiditetima, a u prosjeku se kreće između 30 i 50 posto. Nedavno je prof. dr. Alemka Markotić, ravnateljica Klinike za infektivne bolesti "Dr. Fran Mihaljević" izjavila da dva od tri pacijenta na respiratoru ne prežive.

U Velikoj Britaniji i Italiji smrtnost na respiratoru proljetos je bila gotovo blizu 60 posto.

- Svi zdravstveni djelatnici koji se brinu o pacijentima na respiratorima pod velikim su psihičkim opterećenjem, a i sami pacijenti kad se probude mogu biti poprilično zbunjeni jer ne shvate odmah gdje se nalaze i što se događa - podsjeća Saša Srića.

Sestre i liječnici u jedinicama intenzivnog liječenja koji se o njima brinu, od glave do pete su odjeveni u skafandere, pa, kako je to znao reći doc. dr. Marko Kutleša, pročelnik Zavoda za intenzivnu medicinu Klinike za infektivne bolesti, kad se inficirani probude izgleda im kao da su ih izvanzemaljci oteli.

- Što se tiče stanja u hrvatskim bolnicama, nema razloga za širenje panike, no treba reći da je stopa rasta hospitaliziranih od COVID-a te pacijenata koji su na respiratoru zabrinjavajuća. Ako želimo mirno dočekati jesen, broj oboljelih od koronavirusa mora pasti - zaključuje Srića.

U kolovozu je ministar Vili Beroš izvijestio da će uskoro prema programu EK u Hrvatsku stići još 250 respiratora. To znači da ćemo ih imati preko tisuću.

Iako s ovako visokim temperaturama zraka još i ne pomišljamo na jesen, Saša Srića, pulmolog iz zagrebačke Klinike za plućne bolesti Jordanovac, upozorava da će vrlo brzo krenuti jesenske respiratorne infekcije i poziva građane na cijepljenje protiv gripe.

- Onaj tko u isto vrijeme oboli od gripe i COVID-a može imati teške posljedice. Čak i za one ljude koji su u punom zdravlju, kombinacija te dvije bolesti u isto vrijeme ne bi imala lagan tijek. Gripa također može biti teška akutna bolest s komplikacijama i smrti, iako za razliku od COVID-a ne ostavlja trajne posljedice na plućima, poput plućne fibroze - veli Srića.

Naš sugovornik posebno je zabrinut činjenicom što je u odnosu na 24. kolovoza povećan broj pacijenata koji se liječe u bolnicama od COVID-a za gotovo 100 posto, te što je trenutno 26 najtežih pacijenata na respiratoru, dok ih je koncem kolovoza bilo upola manje - 12.

Ima li osoblja?

- Ima i mladih na respiratoru, nekih čak koji su imali vrlo blage prethodne bolesti, ali se sada bore za život. Na raspolaganju imamo dovoljno respiratora, mislim da ih je ukupno više od 700 u cijeloj zemlji, no pitanje je hoćemo li imati dovoljno zdravstvenog osoblja da skrbi o svim oboljelima ako se nastavi trend rasta - napominje liječnik.

Prema statistikama, većina pacijenata zaraženih COVID-om ima blage simptome bolesti, dok 10 do 15 posto teških slučajeva zahtijeva hospitalizaciju i oksigenoterapiju. Od toga oko pet posto najtežih pacijenata završava na mehaničkom respiratoru, dok srednje teški slučajevi dobivaju kisik u visokim protocima kroz nosne cjevčice. Iako stalno slušamo o respiratorima, na sreću, relativno malo oboljelih je na svojoj koži osjetilo što se događa kad ste priključeni na ovaj stroj.

- Na respiratore se stavljaju najteži pacijenti kojima pluća zbog bilateralne intersticijske pneumonije ne mogu samostalno raditi, odnosno ne mogu više dostaviti dovoljno kisika u krv jer su pluća ispunjena tekućinom i upalom. To onda radi respirator. Bolesniku se pod pozitivnim tlakom preko respiratora dostavlja kisik, a odstranjuje višak ugljičnog dioksida. Kisik koji mi dišemo je u koncentraciji od 21 posto, a na respiratoru se može namjestiti i na 100 posto - objašnjava Srića.

Respirator nije uređaj poput CT-a ili MR-a, gdje ste svjesni svega što se događa. Osobe priključene na respirator su uspavane sve dok ih liječnici ne probude i procijene da mogu samostalno disati. Cijelo vrijeme primaju lijekove protiv bolova, na aparatu za disanje ostaju oko 14 dana, a s njihovim oporavkom treba biti vrlo strpljiv.

- Da bi se oboljeli mogao priključiti na respirator prije ga je nužno intubirati, odnosno uvesti kroz nos ili usta cjevčicu koja je priključena na uređaj. Osoba se prvo uspava, nakon čega mu se cijev provede do plućnih krila. Mnogi znaju reći, kad završiš na respiratoru to je znak da si gotov i da možeš poručiti: "Zbogom! No, nije to baš tako, respirator je spasio živote velikom broju oboljelih – dodaje Srića.

Prilično zbunjeni

Stopa preživljenja pacijenata na respiratoru varira ovisno o dobi pacijenta te njihovim komorbiditetima, a u prosjeku se kreće između 30 i 50 posto. Nedavno je prof. dr. Alemka Markotić, ravnateljica Klinike za infektivne bolesti "Dr. Fran Mihaljević" izjavila da dva od tri pacijenta na respiratoru ne prežive.

U Velikoj Britaniji i Italiji smrtnost na respiratoru proljetos je bila gotovo blizu 60 posto.

- Svi zdravstveni djelatnici koji se brinu o pacijentima na respiratorima pod velikim su psihičkim opterećenjem, a i sami pacijenti kad se probude mogu biti poprilično zbunjeni jer ne shvate odmah gdje se nalaze i što se događa - podsjeća Saša Srića.

Sestre i liječnici u jedinicama intenzivnog liječenja koji se o njima brinu, od glave do pete su odjeveni u skafandere, pa, kako je to znao reći doc. dr. Marko Kutleša, pročelnik Zavoda za intenzivnu medicinu Klinike za infektivne bolesti, kad se inficirani probude izgleda im kao da su ih izvanzemaljci oteli.

- Što se tiče stanja u hrvatskim bolnicama, nema razloga za širenje panike, no treba reći da je stopa rasta hospitaliziranih od COVID-a te pacijenata koji su na respiratoru zabrinjavajuća. Ako želimo mirno dočekati jesen, broj oboljelih od koronavirusa mora pasti - zaključuje Srića.

U kolovozu je ministar Vili Beroš izvijestio da će uskoro prema programu EK u Hrvatsku stići još 250 respiratora. To znači da ćemo ih imati preko tisuću.
