Alergija na kikiriki jedna je od pet najčešćih alergija na hranu koja uzrokuje teške i po život opasne reakcije. Osoba koja je alergična mora izbjegavati kikiriki što je iznimno teško jer ga ima u tragovima u mnogim proizvodima kao što su kolači, sladoledi, žitarice, pekarski proizvodi, namazi, pića... Stoga se neprestano razvijaju metode liječenja i sprečavanja alergije na kikiriki. Nova studija opisuje obećavajuću tehnologiju lijekova koja se temelji na nanočestici koja je u testovima na miševima preokrenula alergije na kikiriki i spriječila njihov razvoj, piše Živim.

- Ako se isti odgovor pokrene kod ljudi, to bi moglo promijeniti živote milijuna osoba alergičnih na kikiriki - kažu istraživači sa Sveučilišta California u Los Angelesu (UCLA). Metoda koju razvijaju temelji se na fragmentu proteina (epitopu), posebno dizajniranom kao antigen, odnosno tvar namijenjena pokretanju odgovora imunosnog sustava, a koji izostavlja dio proteina koji obično uzrokuje alergijsku reakciju. Ideja je da izloženost modificiranoj verziji alergena pomaže trenirati tijelo protiv pune snage stvarne tvari, u ovom slučaju kikirikija.

Kao u cjepivima protiv covida

Ista tehnika korištena je već i u ranijim istraživanjima, ali u ovom su eksperimentu istraživači dodali neke dodatne sastojke: molekulu šećera koja se veže za imunosne stanice koje se bore protiv antigena i korištenje biološke glasničke mRNA za ugradnju uputa u epitop. Dodavanjem mRNA elementa istraživači bi mogli lakše učitati nanočesticu i uklopiti je s više epitopa. Isti pristup koristi se u cjepivima protiv covida, gdje se cijeli protein šiljaka virusa SARS-CoV-2 kodira kako bi se proizveo imunitet.

- Dok je oralna desenzibilizacija sposobna ublažiti anafilaksu alergena kikirikija, dugoročna imunosna tolerancija je traženi cilj. Razvili smo platformu lipidne nanočestice koja cilja na jetru (LNP) za isporuku epitopa alergena kikirikija kodiranih mRNA u sinusoidne endotelne stanice jetre (LSEC). Naši rezultati pokazuju uzbudljivu primjenu mRNA/LNP za liječenje anafilaksije alergena hrane, što omogućuje široku primjenu i na druge elergije – kaže André Nel, imunolog sa Sveučilišta California u Los Angelesu. Kako bi testirali novi tretman alergije na kikiriki, istraživači su nanočestice dali miševima u dvije doze u razmaku od tjedan dana te rezultate usporedili s kontrolnom skupinom koja nije dobila nikakvu terapiju ili je dobila nepotpuni tretman. Pokazalo se da nanočestice smanjuju alergijsku reakciju na kikiriki i povećavaju proizvodnju tvari u tijelu koje bi u budućnosti trebale izgraditi toleranciju na kikiriki. Te su tvari antitijela, citokini (proteini ključni u imunosnom odgovoru) i enzimi (proteini koji pokreću određene kemijske reakcije u tijelu).

Terapija za sve alergije

- Tretman alergije na kikiriki temeljen na mRNA potencijalno bi mogao dovesti i do terapija koje sprječavaju sve vrste alergija, a možda čak i preokrenuti autoimunosna stanja poput dijabetesa tipa 1 - kažu istraživači. No od pokusa u laboratorijima do tretmana koji će zaživjeti u praksi još je dugačak put. Istraživači tim objašnjava da bi klinička ispitivanja na ljudima mogla započeti za tri godine, ovisno o tome kako će ići daljnja laboratorijska istraživanja. No, nanočestice i mRNA tehnologija ima potencijal da pomogne najmanje 4,6 milijuna odraslih i 1,6 milijuna djece koji su pogođeni alergijom na kikiriki. Jedini odobreni tretman alergije na kikiriki traje nekoliko mjeseci i mora se uzimati svakodnevno cijeli život. Zapravo ne sprječava alergijsku reakciju na kikiriki, već smanjuje rizik od teške reakcije. Ranije studije pokazale su i da rano izlaganje kikirikiju pomaže smanjiti rizik od razvoja alergije, što će pomoći oko 20 posto djece da prerastu to stanje, dok će ostalima potrajati cijeli život, , piše Živim.

Alergija na kikiriki jedna je od pet najčešćih alergija na hranu koja uzrokuje teške i po život opasne reakcije. Osoba koja je alergična mora izbjegavati kikiriki što je iznimno teško jer ga ima u tragovima u mnogim proizvodima kao što su kolači, sladoledi, žitarice, pekarski proizvodi, namazi, pića... Stoga se neprestano razvijaju metode liječenja i sprečavanja alergije na kikiriki. Nova studija opisuje obećavajuću tehnologiju lijekova koja se temelji na nanočestici koja je u testovima na miševima preokrenula alergije na kikiriki i spriječila njihov razvoj, piše Živim.

- Ako se isti odgovor pokrene kod ljudi, to bi moglo promijeniti živote milijuna osoba alergičnih na kikiriki - kažu istraživači sa Sveučilišta California u Los Angelesu (UCLA). Metoda koju razvijaju temelji se na fragmentu proteina (epitopu), posebno dizajniranom kao antigen, odnosno tvar namijenjena pokretanju odgovora imunosnog sustava, a koji izostavlja dio proteina koji obično uzrokuje alergijsku reakciju. Ideja je da izloženost modificiranoj verziji alergena pomaže trenirati tijelo protiv pune snage stvarne tvari, u ovom slučaju kikirikija.

Kao u cjepivima protiv covida

Ista tehnika korištena je već i u ranijim istraživanjima, ali u ovom su eksperimentu istraživači dodali neke dodatne sastojke: molekulu šećera koja se veže za imunosne stanice koje se bore protiv antigena i korištenje biološke glasničke mRNA za ugradnju uputa u epitop. Dodavanjem mRNA elementa istraživači bi mogli lakše učitati nanočesticu i uklopiti je s više epitopa. Isti pristup koristi se u cjepivima protiv covida, gdje se cijeli protein šiljaka virusa SARS-CoV-2 kodira kako bi se proizveo imunitet.

- Dok je oralna desenzibilizacija sposobna ublažiti anafilaksu alergena kikirikija, dugoročna imunosna tolerancija je traženi cilj. Razvili smo platformu lipidne nanočestice koja cilja na jetru (LNP) za isporuku epitopa alergena kikirikija kodiranih mRNA u sinusoidne endotelne stanice jetre (LSEC). Naši rezultati pokazuju uzbudljivu primjenu mRNA/LNP za liječenje anafilaksije alergena hrane, što omogućuje široku primjenu i na druge elergije – kaže André Nel, imunolog sa Sveučilišta California u Los Angelesu. Kako bi testirali novi tretman alergije na kikiriki, istraživači su nanočestice dali miševima u dvije doze u razmaku od tjedan dana te rezultate usporedili s kontrolnom skupinom koja nije dobila nikakvu terapiju ili je dobila nepotpuni tretman. Pokazalo se da nanočestice smanjuju alergijsku reakciju na kikiriki i povećavaju proizvodnju tvari u tijelu koje bi u budućnosti trebale izgraditi toleranciju na kikiriki. Te su tvari antitijela, citokini (proteini ključni u imunosnom odgovoru) i enzimi (proteini koji pokreću određene kemijske reakcije u tijelu).

Terapija za sve alergije

- Tretman alergije na kikiriki temeljen na mRNA potencijalno bi mogao dovesti i do terapija koje sprječavaju sve vrste alergija, a možda čak i preokrenuti autoimunosna stanja poput dijabetesa tipa 1 - kažu istraživači. No od pokusa u laboratorijima do tretmana koji će zaživjeti u praksi još je dugačak put. Istraživači tim objašnjava da bi klinička ispitivanja na ljudima mogla započeti za tri godine, ovisno o tome kako će ići daljnja laboratorijska istraživanja. No, nanočestice i mRNA tehnologija ima potencijal da pomogne najmanje 4,6 milijuna odraslih i 1,6 milijuna djece koji su pogođeni alergijom na kikiriki. Jedini odobreni tretman alergije na kikiriki traje nekoliko mjeseci i mora se uzimati svakodnevno cijeli život. Zapravo ne sprječava alergijsku reakciju na kikiriki, već smanjuje rizik od teške reakcije. Ranije studije pokazale su i da rano izlaganje kikirikiju pomaže smanjiti rizik od razvoja alergije, što će pomoći oko 20 posto djece da prerastu to stanje, dok će ostalima potrajati cijeli život, , piše Živim.
