Unatoč brzoj reanimaciji na licu mjesta, dolasku hitne pomoći i na kraju pokušaja oživljavanja na hitnom pedijatrijskom prijemu KBC-u Zagreb, 13-godišnji dječak umro je, liječničkim rječnikom rečeno, od zatajenja rada srca. Njegovo srce nije izdržalo igru "lovice". Sve se to dogodilo prije desetak dana, nakon što se dječak iznenada srušio tijekom igre s prijateljima.

Navodno, kvartovskoj je utrci prethodilo ispijanje veće količine energetskog pića, za koje se sumnja da je moglo potaknuti tu tragediju, ali ipak se ne može sa sigurnošću reći da je to bilo i kobno. Naime, pokazalo se da je dječak imao bolesno srce, odnosno kardiomiopatiju, za koju roditelji nisu znali pa se dječak nije ni liječio. Liječnici drže da je osnovna bolest bila odlučujuća za tragični ishod, unatoč liječničkim naporima, jer je bolesno srce zahtijevalo liječenje, ali kada takav zdravstveni problem ostane neprepoznat, može se dogoditi tragedija kao ova u Zagrebu.

Načini liječenja

"Odmah se počelo s oživljavanjem, prikopčan je bio i na izvantjelesni krvotok tijekom reanimacije, ali, nažalost, nije bilo pomoći", kaže dječji kardiokirurg u KBC-u Zagreb dr. Dražen Belina. Dodaje da je dječak imao tzv. hipertrofičnu kardiomiopatiju, za koju se nije znalo, a koja je i inače najčešći uzrok tzv. iznenadne srčane smrti, odnosno naglog prestanka srčane aktivnosti kod mladih, poglavito sportaša, pa se nerijetko naziva i tihim ubojicom. Moguće je da su tri ili četiri energetska pića, pa trčanje, dodatno potaknuli naprezanje, aritmiju, a onda i prestanak rada srca.

"Premda kardiomiopatija nije česta kod djece, u našoj se bolnici godišnje otkrije barem desetak malih pacijenta. Zato je vrlo važno, poglavito za djecu koja se bave sportom, jedanput godišnje raditi kontrolne preglede, odnosno EKG i ultrazvuk srca. Naime, postoje dvije vrste kardiomiopatije. Jedna je tzv. dilatativna, kod koje se srce jako proširi, što se može vidjeti na rendgenu kao povećana sjena srčanog mišića. No, ovo je dijete imalo hipertrofijsku kardiomiopatiju, kod koje se srčani mišić iznutra jako zadeblja pa nema sjene, zato se na vidi na rendgenu, ali se iznutra smanjuje volumen srca, što se najbolje vidi na pregledu ultrazvukom", objašnjava dr. Belina.

Naglašava također da dilatativna kardiomiopatija najčešće završi presađivanjem srca, a ova hipertrofična nešto rjeđe. Hipertrofična primarno se liječi lijekovima kako bi se spriječilo pogoršanja, smanjili simptomi i rizici. U nekim slučajevima nužno je odstraniti i dio zadebljanja srčanog mišića

Genetska analiza

Naime, iza ove bolesti srca najčešće su genetski problemi, što se dokazuje genetskom analizom.

"Može biti promijenjeno čak između 40 i 60 gena. Genska analiza pokazuje nam koliko je kardiomiopatija kod nekog pacijenta opasna. Inače, ta pretraga ne radi se u Hrvatskoj, već u jednom laboratoriju u Finskoj, a vrlo nam je korisna. Važno je, naime, znati ima li promjene u bjelančevinama "srčanog motora", koje stvaraju kontrakciju srčanog mišića, a kod kardiomiopatije one budu promijenjene. Ima i obiteljskih kardiomiopatija koje se prenose s majke na dijete. No, ima i novih mutacija koje nastaju kod djeteta", objašnjava dr. Belina.

U svakom slučaju, dobro je napraviti vrlo detaljne preglede djece koja se bave amaterskim sportom, barem jedanput godišnje. Talijani su posljednjih godina jako podigli ljestvicu kardioloških pregleda za sportaše općenito, a sve kako bi se izbjegla tzv. iznenadna srčana smrt kod onih kod kojih je bolest bila neprepoznata. Dr. Belina naglašava da tragično preminuli dječak nije imao izgleda, jer je očito splet bolesti i nesretnih okolnosti bio takav da ni brza reanimacija na licu mjesta nije pomogla.

Unatoč brzoj reanimaciji na licu mjesta, dolasku hitne pomoći i na kraju pokušaja oživljavanja na hitnom pedijatrijskom prijemu KBC-u Zagreb, 13-godišnji dječak umro je, liječničkim rječnikom rečeno, od zatajenja rada srca. Njegovo srce nije izdržalo igru "lovice". Sve se to dogodilo prije desetak dana, nakon što se dječak iznenada srušio tijekom igre s prijateljima.

Navodno, kvartovskoj je utrci prethodilo ispijanje veće količine energetskog pića, za koje se sumnja da je moglo potaknuti tu tragediju, ali ipak se ne može sa sigurnošću reći da je to bilo i kobno. Naime, pokazalo se da je dječak imao bolesno srce, odnosno kardiomiopatiju, za koju roditelji nisu znali pa se dječak nije ni liječio. Liječnici drže da je osnovna bolest bila odlučujuća za tragični ishod, unatoč liječničkim naporima, jer je bolesno srce zahtijevalo liječenje, ali kada takav zdravstveni problem ostane neprepoznat, može se dogoditi tragedija kao ova u Zagrebu.

Načini liječenja

"Odmah se počelo s oživljavanjem, prikopčan je bio i na izvantjelesni krvotok tijekom reanimacije, ali, nažalost, nije bilo pomoći", kaže dječji kardiokirurg u KBC-u Zagreb dr. Dražen Belina. Dodaje da je dječak imao tzv. hipertrofičnu kardiomiopatiju, za koju se nije znalo, a koja je i inače najčešći uzrok tzv. iznenadne srčane smrti, odnosno naglog prestanka srčane aktivnosti kod mladih, poglavito sportaša, pa se nerijetko naziva i tihim ubojicom. Moguće je da su tri ili četiri energetska pića, pa trčanje, dodatno potaknuli naprezanje, aritmiju, a onda i prestanak rada srca.

"Premda kardiomiopatija nije česta kod djece, u našoj se bolnici godišnje otkrije barem desetak malih pacijenta. Zato je vrlo važno, poglavito za djecu koja se bave sportom, jedanput godišnje raditi kontrolne preglede, odnosno EKG i ultrazvuk srca. Naime, postoje dvije vrste kardiomiopatije. Jedna je tzv. dilatativna, kod koje se srce jako proširi, što se može vidjeti na rendgenu kao povećana sjena srčanog mišića. No, ovo je dijete imalo hipertrofijsku kardiomiopatiju, kod koje se srčani mišić iznutra jako zadeblja pa nema sjene, zato se na vidi na rendgenu, ali se iznutra smanjuje volumen srca, što se najbolje vidi na pregledu ultrazvukom", objašnjava dr. Belina.

Naglašava također da dilatativna kardiomiopatija najčešće završi presađivanjem srca, a ova hipertrofična nešto rjeđe. Hipertrofična primarno se liječi lijekovima kako bi se spriječilo pogoršanja, smanjili simptomi i rizici. U nekim slučajevima nužno je odstraniti i dio zadebljanja srčanog mišića

Genetska analiza

Naime, iza ove bolesti srca najčešće su genetski problemi, što se dokazuje genetskom analizom.

"Može biti promijenjeno čak između 40 i 60 gena. Genska analiza pokazuje nam koliko je kardiomiopatija kod nekog pacijenta opasna. Inače, ta pretraga ne radi se u Hrvatskoj, već u jednom laboratoriju u Finskoj, a vrlo nam je korisna. Važno je, naime, znati ima li promjene u bjelančevinama "srčanog motora", koje stvaraju kontrakciju srčanog mišića, a kod kardiomiopatije one budu promijenjene. Ima i obiteljskih kardiomiopatija koje se prenose s majke na dijete. No, ima i novih mutacija koje nastaju kod djeteta", objašnjava dr. Belina.

U svakom slučaju, dobro je napraviti vrlo detaljne preglede djece koja se bave amaterskim sportom, barem jedanput godišnje. Talijani su posljednjih godina jako podigli ljestvicu kardioloških pregleda za sportaše općenito, a sve kako bi se izbjegla tzv. iznenadna srčana smrt kod onih kod kojih je bolest bila neprepoznata. Dr. Belina naglašava da tragično preminuli dječak nije imao izgleda, jer je očito splet bolesti i nesretnih okolnosti bio takav da ni brza reanimacija na licu mjesta nije pomogla.
